You get this End either by choosing to kill Akiha or if Akiha's affection level isn't high enough.

Though, as you why it phrases the choice as "Keep my promise," I really couldn't tell you, because, while Akiha tries to make Shiki promise to kill her if this situation comes up, Shiki actually refuses to make that promise.
