First we take down Misery. Then the Doctor, then the Doctor after having a large dose of red crystal... THEN WE SHOOT THE CORE! Pay attention to the annotations because that's where the real help will be... I recommend Spur for Neutral ending, Snake for good, and Any for Bad... Snake helps GREATLY in Sacred Grounds.
