Episode 3 Map 8, Dis, on Ultra Violence difficulty with 100% kills. The final level of the original game, and the final boss is the Spider Mastermind who walks around the map very slowly and has a devastating chaingun attack. There are no secrets on this level. Since this is the end of the original game, there's a nice little closing cutscene for you to enjoy, and personally I still find it kind of creepy.
